Hardcopy Transformation Award Applications | Transformación Impresa Solicitudes de Premios

We are accepting hardcopy Transformation Award Applications 

Due to the COVID-19 crisis, we strongly recommend that applicants submit their Transformation Award application using this online form. 

For applicants who have access issues and are unable to complete the online form, Leeway will still be accepting hard copy applications. However, before deciding to submit a hard copy application, please know the following information: 

  • If you choose to submit a hard copy application, we strongly encourage you to mail the application to Leeway’s office at 1315 Walnut Street, Suite 832, Philadelphia, PA 19107 
    • If you mail your application, it must be postmarked May 15th  
  • Leeway’s physical office is located next to the Holiday Inn Express, a hotel that has become a COVID-19 quarantine site. Because of this, the city has closed Walnut Street between 13th and Juniper Streets.  
    • For this reason, we cannot recommend physically dropping off your application. 
    • If you choose to drop off your application at our office, please know that only the Sansom Street entrance, between 13th and Broad Street, of The Philadelphia Building is open to the public. 
    • Leeway’s staff is working remotely - and will not be present in office to physically receive your application on the deadline date.

Sí aceptamos solicitudes impresas para el Premio Transformación

Debido a la crisis del COVID-19, es altamente recomendable presentar su solicitud para el Premio Transformación, mediante este formato en línea. 

Para aquellas personas solicitantes que tienen problemas de acceso y no pueden rellenar el formato en línea, Leeway seguirá aceptando solicitudes impresas. No obstante, antes de tomar la decisión de presentar una solicitud impresa, favor de tener en cuenta lo siguiente:

  • Si decide presentar una solicitud impresa, le recomendamos encarecidamente enviar la solicitud por correo a la oficina de Leeway, 1315 Walnut Street, Suite, 832, Philadelphia, PA 19107
    • Si envía su solicitud por correo, es indispensable que tenga matasellos del 15 de mayo a más tardar como fecha límite.
  • La oficina de Leeway se encuentra al lado del hotel Holiday Inn Express, mismo que se ha convertido en un sitio de cuarentena del COVID-19. Por tanto, el ayuntamiento ha cerrado la calle Walnut entre la calle 13 y Juniper.
    • Por esta razón, no recomendamos entregar su solicitud físicamente.
    • Si decide entregar su solicitud en la oficina, tenga en cuenta que solo está abierta la entrada de la calle Sansom, entre la calle 13 y Broad, por el edificio Philadelphia.
    • El personal de Leeway está trabajando de manera remota, y no estará presente en la oficina para recibir su solicitud en la fecha límite.
